I understand images and videos may be used for coaching, safeguarding records, and promotional purposes including social media, website, and marketing. I understand I can withdraw consent at any time by contacting Gloves Up.

I confirm I am the parent/legal guardian of the participant and give permission for them to take part in Gloves Up boxing sessions. I understand physical activity carries risk of injury, confirm my child is fit to participate, and agree they will follow coach instructions and safety guidelines.*
